TELEHEALTH RETURNS AS OF JULY 1, 2023 FOR RECERTIFICATIONS FOR OUR CURRENT PATIENTS.
NEW PATIENTS AND TRANSFERS MUST BE SEEN IN PERSON THE FIRST TIME PER FLORIDA STATE LAW.

In summary,

RETURNING PATIENTS may recertify virtually. NEW or TRANSFER patient would have to be seen in-person in the Fort Lauderdale office.

If you are a returning patient of Dr. Shaw's or CGS Physician Services, LLC, WELCOME BACK! We realize you have a multitude of choice when it comes to your health care and we greatly appreciate your trust and satisfaction with our services.


EXCEPTIONS can be done EITHER virtually if you have been seen by Dr. Shaw at least once before OR in person. Exceptions are available for qualifying conditions that are of a chronic, progressive nature in order to qualify for an increase greater than the current state limits.
