Cancellation Policy

Emails and voicemails will not be considered as confirmed cancellations. FEES: Cancellation up to 24 HOURS BEFORE your scheduled appointment is FREE. CGS Physician Services, LLC will not charge any fee. Cancellation WITHIN 24 hours of your scheduled appointment carries a fee of $25. CGS Physician Services, LLC will assess a $25.00 office cancellation fee if the appointment is cancelled LESS THAN 24 HOURS of the scheduled appointment date OR IF AN APPOINTMENT IS MISSED. Chargebacks: If a customer initiates a chargeback, CGS Physician Services, LLC will assess a $50.00 processing fee for each individual chargeback.
